Every signal in SNF Shark traces back to a specific public dataset — mostly CMS, plus HUD-FHA for the real-estate debt, FEMA for flood zones, and the state Medicaid agencies for reimbursement. This page shows when we last refreshed each source, when the publisher last put out new data, and the next refresh date (exact where the publisher keeps a calendar, estimated from cadence otherwise). It publishes nurse staffing hours in three distinct families that are easy to confuse: REPORTED hours (what was actually staffed), CASE-MIX hours (the hours a facility's resident acuity implies — this is the case-mix index rescaled, not a measure of staffing), and ADJUSTED hours (reported hours normalised for that acuity, which is what CMS uses for the staffing star). | FEMA National Flood Hazard Layer (Flood Zones) FEMA flood zone at each facility's mapped coordinates — a point-in-polygon lookup of the CMS-published latitude/longitude against FEMA's digital flood maps, with the FIRM panel's effective date and a 150 m near-SFHA proximity check for facilities outside an SFHA. Special Flood Hazard Areas (zones A/AE/AH/AO/A99/AR/V/VE, the 1% annual chance floodplain) trigger mandatory flood insurance on federally backed loans, which is why the facility hero flags SFHA and nothing else. Where the point falls on a zone boundary or in a seam between map panels, we report an adjacent zone from within 50 m, preferring the SFHA side — the conservative read for screening. Screening information only, not a flood zone determination: lenders obtain a Standard Flood Hazard Determination (FEMA Form 086-0-32) from a qualified vendor, and individual buildings can be removed from the SFHA by a Letter of Map Amendment or Revision (LOMA/LOMR-F) that map polygons do not reflect. A point lookup at a geocoded address is also not a building-footprint reading — a campus can straddle a zone edge. “Not mapped” means FEMA publishes no digital flood map for the location (some communities have paper-only maps), not that risk is low; Zone D means FEMA has not studied the area. Dates marked est. are computed from each dataset's typical cadence — the publisher posts no specific next-refresh date for those (HCRIS, PBJ, PECOS, MDS Frequency, Chain Performance, Texas HHSC, HUD-FHA, FEMA NFHL). For all others, the date is the one CMS has on its calendar. FEMA revises flood maps on a rolling basis with no release schedule at all — its cadence and next-expected date describe our own quarterly re-check, not a FEMA publication. HCRIS and HUD-FHA publish no catalog at all; their "last published" is read from the Last-Modified header on the file itself, which for HUD trails the data's as-of month-end by about a week. Texas HHSC is the one row whose status is not a refresh-date comparison. HHSC publishes no catalog and no file index we can read, so its "last published" can only be the Last-Modified stamp on workbooks we already hold — a date that cannot, by construction, tell us a file we have never downloaded exists. Its status instead answers whether we hold the scorecard quarter HHSC should have out by now: QIPP quarters close Nov/Feb/May/Aug and the scorecard has historically appeared two to three-and-a-half months later, so we allow four months from quarter close before calling ourselves behind. The "next expected" date for that row is that four-month deadline for the following quarter.
Beyond CMS, SNF Shark ingests per-facility Medicaid reimbursement rates published by individual state Medicaid agencies. Coverage is rolling out state by state — each facility's rate appears on its Reimbursement tab. A weekly sweep checks every state's source for newly published rate files (last ran yesterday); "Rates effective" is the vintage of the newest file we hold.

The Demographics tab's Market Supply card (bed supply, market occupancy) is computed from CMS bed & census data (the Provider Information row above); only the demographic and CON figures use the reference sources listed here. Forward population projections and household net-worth estimates are intentionally not published — they aren't available from a free public source at this grain.
On top of the base Medicaid per diem, Illinois' 2022 rate reform (PA 102-1035) pays several per-facility supplemental flows and levies a provider assessment. SNF Shark ingests the per-facility figures Illinois HFS publishes and shows them, netted, on each Illinois facility's Reimbursement tab. Provider-assessment dollars are estimated (published tax rate × occupied bed days); all other figures are as published by HFS.
